Useful documents

  • Official identification
    (Identity card, passport)
  • Insurance Card
    (personal health insurance card)
  • Notice to EU citizens:
    Residents of EU countries who are insured in a statutory health insurance scheme require a European Insurance Card. In case of an emergency your insurance company should be contacted, so that a provisional replacement certificate can be issued and sent by fax.
  • Blood group card
  • Allergy card (for allergy sufferers)

Useful medical information

We are not usually aware of  the circumstances of the accident or of the medical history of the incoming patient. If possible try to recall the significant incidents of the accident or sickness.

  • What happened exactly?
  • How did the health problem become apparent?
  • Are there other diseases?
  • Were there diseases in the past?
  • Are medicines being taken? 
  • If yes, which medicines and in what doses?
  • Is the patient an allergy sufferer?
